Key things to know
📅 Two-year lookback
Your 2026 IRMAA is based on your 2024 tax return — the most recent year the SSA can access from the IRS.
⚠️ Cliff effect
Earning even $1 over a bracket threshold triggers the full surcharge for that tier — potentially $1,000+ more per year.
📝 MAGI defined
MAGI = Adjusted Gross Income (Form 1040, Line 11) + tax-exempt interest (Line 2a), including municipal bond income.
🔁 Appeal (SSA-44)
You can appeal IRMAA due to a qualifying life event (retirement, divorce, death of spouse) using Form SSA-44.

How IRMAA works
IRMAA is an income-related surcharge added to Medicare Part B and Part D premiums when income is above certain thresholds.
- 2026 IRMAA usually looks at 2024 MAGI.
- Part B IRMAA is added to the Part B premium.
- Part D IRMAA is paid in addition to the drug plan premium.
Ways to reduce or avoid IRMAA
- Plan Roth conversions before Medicare lookback years when possible.
- Manage taxable IRA withdrawals and capital gains.
- Use Form SSA-44 if a qualifying life-changing event lowered income.
- Review income before age 63 if enrolling in Medicare at 65.
